The Health Care and Fitness Journal was designed during a time in my life when I was forced to pay closer attention to my health, as well as the care I was receiving from my physicians. Its initial purpose was to simply keep me organized by providing a central location for my health and fitness information. After I began using the Journal, I realized it was more than an organizational tool. It actually held me more accountable, helped me to become a better manager of my health, and empowered me to take charge of my healthcare!
For example, in a designated section of the journal, I prepare for my doctor visits by listing my questions and concerns prior to each visit. Early preparation allows communication to flow smoothly between me, healthcare staff, and my doctors resulting in improved quality care.
One of my favorite sections of the Journal is the section containing My Progress Report. This section is designed to call you to TAKE ACTION by setting your health goals and keeping them in a place where you can see them daily. Writing down my goals and keeping them in front of me reinforces my commitment to myself to exercise, and to make healthier nutritional choices.
